The top of the ranking is led by Ireland at 98% (rank 1), followed by Rwanda at 96% (rank 2). Four countries share rank 3 with 95% coverage: Botswana, Namibia, Sweden, and Zimbabwe. Eswatini, Lesotho, the United Kingdom, and Zambia all sit at rank 7 with 94%. Burundi, Germany, and Saudi Arabia share rank 11 at 93%. High-income European countries and sub-Saharan African countries with a high HIV burden both appear prominently at the top of the ranking, with values clustered between 90% and 98% across the top 20 positions.
The middle of the ranking shows considerable spread. Canada ranks 61st at 76%, while Brazil ranks 71st at 73% and India ranks 72nd at 72%. A notable counterintuitive placement is Saudi Arabia ranking 11th at 93%, higher than Canada (61st, 76%) and the United States is absent from the dataset entirely. At the lower end, Pakistan ranks 143rd at 16% and Afghanistan ranks 144th at 11%. Several large countries by population appear in the bottom half: Indonesia ranks 132nd at 41%, Bangladesh ranks 132nd at 41%, and the Philippines ranks 135th at 40%. Congo, Rep. ranks 136th at 34%, dropping sharply from neighboring Central African Republic at rank 88 with 66%.
Several data limitations apply to this metric. Coverage percentages depend on the accuracy of both the numerator (people on ART) and the denominator (estimated total population living with HIV). Denominator estimates for HIV prevalence carry uncertainty, particularly in countries with limited surveillance infrastructure, which can cause coverage figures to appear artificially high or low. Countries with very small HIV-positive populations may show extreme percentages from minor changes in case counts. Reporting practices vary by country, and some figures reflect modeled estimates rather than direct counts.
